I believe the poem is about two people in an intimate relationship where the sense of separateness has started fading. But one partner is still not ready to surrender totally to this merger and the other partner is urging him/her to just jump into this blissful togetherness. Thus the poem goes-

We have our own separate bodies (and lives), but perhaps they are not our own anymore (दोन हे आहेत पेले आपुले नाही जणू), because at present we are so entangled in each other, it is so confusing as to where your existence begins and where mine ends (कोणता आहे तुझा अन कोणता माझा म्हणू)

These two streams of separate existence have merged with each other (दोन धारा एक झाल्या या प्रवाही तू नि मी), this merger has enriched both of us and the sense of separate bodily existence has ceased to exist now (पूर येतो वाढणारा आपुली नाही तनू)

We have these two separate bodies but I have given up on mine totally (दोन हे आहेत पेले एक माझा संपला), but you still have not been able to jump into this stream of joint existence and still trying to look at this relationship with objectivity of an outsider, (झाकुनी अर्धाच पेला टेकिली का वर हनु) (I love this reference to resting of chin, it just so beautifully talks about looking at a situation objectively)

I urge you to just throw in your existence into this relationship and merge into mine (दे तुझा झोकून दे तो आणि माझा घे तुला) and enjoy the bliss and the ecstasy of togetherness (होत जागी रानगीते हृदय लागे गुणगुणू)
